Frailty syndrome is frequently associated with heart failure (HF). It is a progressive condition associated with a poor prognosis but is potentially reversible. The study presented here aims to evaluate the feasibility of telephone-based frailty screening conducted by a registered nurse among individuals with HF who are being monitored via telemedicine. The goal is to enable early intervention in order to reduce the burden of frailty and improve patients' quality of life.
