Brugada syndrome is an inherited cardiac channelopathy associated with sudden cardiac death, typically diagnosed in middle-aged individuals. Fever is a well-known trigger for malignant ventricular arrhythmias in predisposed patients. We report the case of a 73-year-old man without cardiovascular risk factors or previous syncope who experienced ventricular fibrillation-related cardiac arrest triggered by fever as the first clinical manifestation of Brugada syndrome. This case highlights the pivotal role of fever in unmasking high-risk ECG patterns and challenges the assumption of age-related protection.
