A 14-year-old castrated male spitz dog was presented with intermittent coughing. Physical examination revealed bradycardia and a grade 3/6 left apical systolic murmur. Thoracic radiographs demonstrated severe bulging of the aortic arch with focal dorsal deviation of the trachea. Echocardiography revealed marked dilation involving the sinotubular junction and extending into the ascending aorta without discernible physiologic causes, with moderate aortic regurgitation and mild mitral regurgitation secondary to myxomatous degeneration. Electrocardiography showed a 2:1 second-degree atrioventricular block. Idiopathic thoracic aortic aneurysm was presumptively diagnosed. The dog was managed medically with pimobendan, theophylline, and famotidine. Clinical signs and electrocardiographic abnormalities improved, and the dog remained stable for 13 mo. Key clinical message: This case report demonstrates the diagnostic and therapeutic challenges and clinical relevance of idiopathic thoracic aortic aneurysm in dogs. Anévrisme idiopathique de l’aorte thoracique chez un chien présentant un bloc auriculo-ventriculaire 2:1Un chien Spitz mâle castré de 14 ans a été présenté pour une toux intermittente. L’examen clinique a révélé une bradycardie et un souffle systolique apical gauche de grade 3/6. Les radiographies thoraciques ont mis en évidence un bombement important de la crosse aortique avec une déviation dorsale focale de la trachée. L’échocardiographie a révélé une dilatation marquée de la jonction sinotubulaire s’étendant à l’aorte ascendante sans cause physiologique identifiable, avec une régurgitation aortique modérée et une régurgitation mitrale légère secondaires à une dégénérescence myxomateuse. L’électrocardiogramme a montré un bloc auriculo-ventriculaire du deuxième degré de 2:1. Un anévrisme idiopathique de l’aorte thoracique a été présumément diagnostiqué. Le chien a été traité médicalement avec du pimobendane, de la théophylline et de la famotidine. Les signes cliniques et les anomalies électrocardiographiques se sont améliorés, et l’état du chien est resté stable pendant 13 mois.Message clinique clé :Ce cas clinique illustre les difficultés diagnostiques et thérapeutiques ainsi que la pertinence clinique de l’anévrisme idiopathique de l’aorte thoracique chez le chien.(Traduit par Dr Serge Messier).
