In chronic heart failure (CHF), serum uric acid (SUA) levels rise primarily due to increased production, partially due to decreased excretion, or both elevated uric acid levels are a sign of developing heart failure and cardiac dysfunction due to oxidative stress and enhanced xanthine oxidase activity. The aim of this study was to see the association between admission serum uric acid levels and chronic heart failure and to find its significance in predicting short term outcome (mortality and rehospitalization). This prospective cohort study was done in the Department of cardiology, National Heart Foundation (NHF) Hospital and Research Institute from April 2018 to March 2019. A total sample of 148 patients with Chronic Heart Failure was included after considering inclusion and exclusion criteria that had on admission Serum uric acid level measured and follow-up done after 30 days over telephone or hospital follow-up visit as appropriate. Study patients were divided into two groups according to Serum uric acid level Group I (SUA in men <7mg/dl) (SUA in women <6mg/dl), Group II (SUA in men ≥7mg/dl), (SUA in women ≥6mg/dl) Baseline characteristics. Serum uric acid was measured and complete follow up was done on 148 patients. Hyperuricemia was found to be significantly higher in patients with Chronic Heart Failure and the level of serum uric acid (SUA) rise had a high correlation to the severity of heart failure. Elevated serum Uric Acid levels were associated with poorer New York Heart Association (NYHA) functional class, proving that hyperuricemia predicts the severity of cardiac failure. Patients with higher uric acid levels were associated with adverse outcomes and poor prognosis in the form of increased rehospitalization rates and 30-day mortality rates. Individuals with lower serum uric acid levels at admission had better short-term outcomes than patients with higher serum uric acid levels.
