儿童射血分数保留的心力衰竭（HFpEF）病因谱复杂、表型高度异质，常与先天性心脏病、心肌病、遗传性综合征、肺动脉高压及肥胖代谢异常等因素相关，诊断和治疗均不能简单套用成人经验。近年来，年龄分层的心功能评估、心脏磁共振成像、新型生物标志物及机制导向治疗为儿童HFpEF的识别和干预提供了新思路。现围绕儿童HFpEF的诊断评估、分子病理机制及机制导向治疗进展进行综述，以期为分层诊疗和未来研究提供参考。.
