A 7-month-old infant with Tetralogy of Fallot physiology showed disproportionate left heart dilatation. Multisite saline contrast echocardiography demonstrated immediate left sided atrial opacification, suggesting anomalous systemic venous return. Cardiac CT confirmed total anomalous systemic venous connection to the left sided atrium with left isomerism. Early recognition enables timely surgical correction and improved outcomes.
