Each year, in Belgium and Europe, thousands of contraceptive implants are inserted, representing an effective and long-lasting method of contraception. Their ease of use and prolonged duration of action make them a preferred choice for many women. However, although complications are rare, they do exist. Among these, implant migration is one of the most concerning. Its incidence remains extremely low, estimated at approximately 1 case per 100,000. In most cases, this migration is asymptomatic, but it can sometimes cause alarming symptoms. These may include dyspnea, hemoptysis, or, in rare cases, potentially serious vascular injuries. Once the implant becomes endothelialized, its removal becomes more complex. In such situations, therapeutic management must be tailored to the patient's clinical profile and the precise location of the implant. Options range from active surveillance to complex surgical interventions, especially in cases where attempts at removal using endovascular techniques, such as interventional radiology, have failed. Chaque année, en Belgique et en Europe, des milliers d’implants contraceptifs sont posés, constituant une méthode efficace et durable de contraception. Leur facilité d’utilisation et leur longue durée d’action en font un choix privilégié chez de nombreuses femmes. Cependant, bien que les complications soient rares, elles existent. Parmi celles-ci, la migration de l’implant vers la circulation pulmonaire est l’une des plus redoutées. Son incidence reste extrêmement faible, estimée à environ 1 cas sur 100.000. Dans la majorité des situations, cette migration est asymptomatique, mais elle peut parfois engendrer des symptômes préoccupants. Cela peut entraîner des symptômes tels que la dyspnée, l’hémoptysie ou, dans de rares cas, des lésions vasculaires potentiellement graves. Une fois l’implant endothélialisé, son retrait devient plus complexe. Face à une telle situation, la prise en charge thérapeutique doit être adaptée au profil clinique de la patiente et à la localisation précise de l’implant. Les options varient de la surveillance active, jusqu’à des interventions chirurgicales complexes, surtout en cas d’échec des tentatives de retrait par des techniques endovasculaires comme la radiologie interventionnelle.
